Oak Plains Academy has a unique opportunity for a Masters' Level Case Manager to join our team of professionals who support our youth living in Oak Plains Academy foster homes across Middle Tennessee. The ideal candidate will enjoy the benefits of a flexible schedule, diverse responsibilities, and smaller caseloads which allows more focused time to build stronger connections with the youth served.
Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy offers psychiatric residential treatment services to children and adolescents ages 5-17, struggling with emotional and behavioral issues. The Oak Plains Academy campus features a full-size gymnasium, separate academic building, and four dormitories. Our wooded campus provides an excellent opportunity for outdoor recreational activities such as fishing, hiking, softball and basketball. Additionally, Oak Plains Academy provides therapeutic treatment and intensive case management support to youth, from infant to 17 years old, residing in our Foster Homes. The Foster Home Case Manager will be responsible for providing in home trauma based therapy and intensive case management and coordination of services to youth and adolescents.
