Manager of Case Management (RN) - LTC CHOICES - Davidson, Sumner and Wilson Coun

Purpose of Job: The Manager of Case Management will manage the TN Long Term Care Choices care management team, including Registered Nurses and Social Workers, with responsibility for clinical operations and medical management activities across the continuum of care. Also, in collaboration with the Health Services Director, the Manager will work to ensure compliance with contract requirements as it relates to care coordination and related activities.
